Final Report Format for Grants of $25,000 or more

All grantees are required to submit a 3-4 page final report one year after receipt of the grant. It can be sent as an electronic attachment to your e-mail or sent by regular mail to the office of the Samuel N. & Mary Castle Foundation. If more time is required to complete the project and report, please contact the Executive Director.

Every report on a grant over $25,000 should include the following:

I. A project or program budget as submitted in the original proposal and a final program or project budget showing actual expenditures. Any budget variance should be explained.

II. The rest of your final report should include a narrative response to the following questions regarding your capital improvement, scholarship, your program or your general project grant.


  1. Briefly restate the plan outlined in the original request. Were any modifications to the plan necessary? If so, please describe. What effects have modifications had?
  2. Briefly restate the measurable outcomes you outlined in the original request. What progress have you made toward achieving these outcomes during this period? If outcomes have changed from those originally proposed, please explain why and what you learned. What has been the effect of your project on the community?
  3. What methods or strategies are being used to gather data on the program? Are there any findings of interest as of this point? If so, please elaborate.
  4. Lessons learned: what do you consider to be the greatest strengths(s) of the program? What do you consider to be the most important concerns(s) – apart from finances – currently facing the program?
  5. Has this grant been instrumental in attracting additional resources in the form of people, goods, services, or publicity? If so, describe.
  6. The Castle Foundation is flexible, so if you wish to substitute an oral report, a video, or reports from beneficiaries of the project grant, please let us know. Community should be he center of your proposal, your project and your final report.
  7. If your organization learned important lessons from the project or program, will you share your learning with the community and other organizations?

Please remember that not sending the required report disqualifies the organization from future grant application consideration.
